Immunization Requirements

Upon enrollment, state law mandates that you must provide the school with a record showing that your child has received at lease one dose of the following immunizations (Parents must then follow up to complete the required vaccinations), or a waiver must be signed:

IMMUNIZATIONS

AGES 4-6

AGES 7-18

Diphteria, Tetanus, Pertussis

4 doses DTP or DTaP

One dose must be administered on or after the 4th birthday.  If not, a booster dose of DTP is required.

4 doses D & T OR 3 doses

Td if #1 given on or after 7th birthday.  One dose must be administered within the past 10 years, or a booster dose is required.

Polio

3 doses are required. 

If dose 3 is given prior to the 4th birthday, a booster dose is required.

3 doses are required.
Measles, Mumps, Rubella

2 doses are required. 

Dose #1 must be administered after the 1st birthday.  The 2nd dose must be administered a minimum of 28 days after the 1st dose, or a booster is required.

2 doses are required. 

The 2nd dose must be administered a minimum of 28 days after the 1st dose, or a booster is required.

Hepatitis B

3 doses are required. 

Dose #2 administered 1 month after the 1st dose.  Dose #3 administered 6 months after the 1st dose.

3 doses are required. 

Dose #2 administered 1 month after the 1st dose.  Dose #3 administered 6 months after the 1st dose.

Varicella*
(Chicken Pox)

*Reliable history of disease is acceptable in lieu of immunization
1 dose is required on or after the first birthday, but prior to the 13th birthday.  If immunized on or after the 13th birthday, 2 doses are required.1 dose is required on or after the first birthday, but prior to the 13th birthday.  If immunized on or after the 13th birthday, 2 doses are required.
Hib

Pre-school programs only
1 dose is due at 2-3 months.  The 2nd dose is due at 4-14 months.  1 dose after or on 15th month or completed series earlier.
 
Pneumococcal Conjugate

Pre-school programs only

5 doses are required.

Dose #1 is due at 2-3 months.  The 2nd dose is due at 4-5 months, the 3rd dose is due at 6-14 months.  Dose #4 is due at 15-23 months.  The final 5th dose is due at 24 months of age.

 

For Kindergarten students only, proof of a vision screening is also required.
